Need to Make Changes to Your Benefits?

 

Life happens, and sometimes your benefits need to adjust to keep up. Whether it's a new baby, marriage, or other major life events, here's what you need to know and do:

 
 

A Qualifying Life Event (QLE) is a significant change in your life that allows you to make changes to your benefits outside of the annual open enrollment period.

Qualifying Life Events include: 

  • Birth, Marriage, Divorce, Loss of Coverage: Your coverage change takes effect on the event date.
  • Death, Gaining Coverage, Dependent Reaching Age Limit: Changes are effective at the end of the month when the event occurred.

  • Premium Adjustments: If your event changes your coverage level, we’ll adjust your premiums. For example, if you have a baby mid-month, we’ll update your coverage and collect any necessary differences in your premium on your next paycheck.
  • Rate Changes: Your monthly premium may change depending on your new coverage. Check the premium rate sheet for details.
  • Catch-Up Deductions: Since we prepay for most insurances, there may be a need to recoup missed premiums or adjust for differences in premiums. For example, if you have a baby on April 14th,  your deductions may not start until after the birth, but you will need to pay the premiums for the month of April.
  • Insurance Cards: It may take a few weeks to receive your new insurance card. However, your coverage will be effective retroactively from the date of the qualifying event.

You have 30 days from the event to provide the required coverage change forms and supporting documents to nohrmbenefits@lsuhsc.edu

  • Forms:
  • For medical changes, fill out the GB01 Enrollment Form and include the required documents.
  • For Dental/Vision changes, use the forms available on our website.
  • For all other benefit plans, please contact the Benefits Team: nohrmbenefits@lsuhsc.edu
  • Documentation:
    • Marriage: Spouse’s Social Security Card & Marriage License
    • Birth: Hospital Birth Letter (Submit official Birth Certificate and Social Security card later)
    • Death: Death Certificate
    • Loss or Gain of Coverage: Proof of coverage change, Social Security cards for dependents being added or removed
